Release and Waiver of Liability

I, the Participant, or the parent/guardian of the Participant, understand that Elevate Spectrum, Inc. including its agents, employees, contractors, officers, directors, successors, and assigns (collectively referred to as “Elevate Spectrum”), makes every effort to operate in a safe, supportive, and responsible manner.

I acknowledge that participation in Elevate Spectrum’s services, activities, or programs may involve certain inherent risks, including but not limited to injury, illness, or, in rare cases, death. I voluntarily assume full responsibility for any and all risks associated with participation.

In consideration for the Participant’s involvement with Elevate Spectrum, I hereby waive, release, and forever discharge Elevate Spectrum from any and all claims, liabilities, demands, or causes of action of any kind—whether known or unknown—arising out of or related to any injury, loss, or damage sustained as a result of participation, including those arising from the negligent acts or omissions of Elevate Spectrum.

Furthermore, I agree to indemnify, defend, and hold harmless Elevate Spectrum from and against any claims brought by or on behalf of the Participant or any third party related to the Participant’s involvement with Elevate Spectrum.
